Athletic Clearance

Athletic Clearance registration for the upcoming school year 2026/2027 will open up May 15, 2026! 

Athletic Clearance 2026-2027 Due Dates

 
  • Fall Season: June 4, 2026
  • Winter Season: October 9, 2026 
  • Spring Season: January 8, 2027

 

 

We welcome all sport seasons to turn in athletic clearance forms early.

Please read the Athletic Clearance Guide below to be sure to turn in all required forms. Home Campus registration is required for an athlete to be cleared to try out for a Poway sport team. If trying out for multiple sports, be sure to include them on your initial registration.

Health History and Physical Forms

  • Print blank Health History and Physical forms by clicking HERE 
  • Take the completed Health History form and blank physical form to the doctor (MD, DO, PA or NP is acceptable).  The doctor must complete, sign, date, and stamp the physical form. If you are unable to get an appointment with your child's pediatrician, please visit your nearest CVS Minute Clinic.   

*Starting 26/27 physicals may not be done by chiropractors 

Online Registration 

  • Complete the online  AthleticClearance.com registration for the 2026-27 school year. Follow the Instructions for Completing the Athletic Clearance. Video/Handout. IMPORTANT STEP FOR RETURNING ATHLETES:   Log in to your EXISTING profile from the previous year (do NOT create a brand new account).  Then select 26/27 and follow the prompts.   Click the dropdown under "Choose Existing Student" and select the student name. Following this step ensures a physical from last year will auto-populate in a student's 26/27 profile. Please do NOT create multiple Home Campus profiles for your student.  You may add additional sports to their existing profile by checking the 'add sport' button. 

*If you have a physical exam on file, upload a blank physical stating its on-file. 

Submission 

  • An email from Home Campus with the status "Cleared" will be sent to the account's associated email when it has been reviewed and cleared. Home Campus will reject incomplete physicals and send you an email outlining the reasons and corrective actions.



Late Submissions 

An incomplete registration could delay the clearance further, so please make sure you make it all the way to the Confirmation Message. An Athletic Clearance registration completed after the due date may delay and/or prohibit the athlete from trying out. A late submission can extend the length of time to review, depending on time allotted outside of responsibilities to the in-season sports and school breaks.

Obtain a Physical

Print the Poway's Athletic Physical forms (3 pages). Complete the health history (two pages), then take to your physician with the Physical Examination form for them to complete. Physicals are good for ONE year from date of examination and cannot expire during the sports season. {Must be valid prior to the season and valid through the conclusion of the season.} Make sure all line items {‘yes’ explanations, dates, signatures, etc.} are completed before you leave the doctor; otherwise omissions may cause delays. Physicals must now be completed by one of the following California Licensed health care professionals: Physician (MD/DO), Physician’s Associate (PA-C), or Nurse Practitioner (NP). *Starting 26/27 physicals may not be done by chiropractors 

Check Your Cleared Status

Log into your AthleticClearance.com account. Uncleared status indicates your registration is pending review; and if you have Denied status, please check your email associated with AthleticClearance.com account for action items. The email subject will be titled 'Student Clearance' and the email message will be from Home Campus.

The Athletic Clearance must be renewed each school year. Only Poway Athletic Clearance forms are accepted.
